This page contains information about how to use your existing membership, including useful links and documents that may be needed from time to time.
*If you have not signed up as a member yet, please contact the clinic (call or text at 830-222-6296) to discuss enrollment. This page is for actively enrolled members!
Getting oriented to all the technology
Here is a summary of the various tools used in your care. See the sections lower in this webpage for additional information about each of these items.
• Appointments can be scheduled through Calendly.
• Membership payments are made using Hint. You can request access to the Hint self-service portal here.
• The electronic medical record system (EMR) we use is called Elation. Elation provides access to your patient portal (called Elation Patient Passport). You will need to first obtain an invitation to access your Patient Passport. Please contact the office and we will get it sent out to you.
• Encrypted communications are handled by the Spruce app. It can be downloaded here.
• Commonly used forms (such as records request forms) are found in a section below.

If you need to schedule an appointment outside of the available times listed on the Booking Site (especially for urgent or same-day concerns), please contact Dr. Rob and we can find a time that works for you.
hint: paying invoices and making changes to membership status
If you need to review or make any changes to your membership, get access to your Hint Self-Service Portal HERE. You will receive an email containing a link to log in.
The Hint self-service portal allows you to:
- view your membership details
- pay and manage invoices
- manage payment sources such as credit card or bank account
- view and sign membership agreements
- update contact information (phone number, email address, mailing address)
elation patient passport (Patient Portal)
Our electronic medical record (EMR) system is called Elation. This EMR allows you to set up a patient portal (called Elation Patient Passport), in which you can view your clinical information and visit summaries. Please contact Dr. Rob to request an invitation to Elation Patient Passport. This invitation will go to the email address we have on file for you. It would be a good idea to periodically (every 6 or 12 months) check the clinical information we have on file for you to make sure that your list of medical problems and medications is up to date.
Spruce communications app
Spruce is the app that we use to send encrypted messages in a HIPAA-compliant manner. We can use it to send secure text messages, photos, and PDF files. It is also the preferred tool we use for video visits. It is recommended to download the Spruce app to your smartphone, but it can also be used through a web browser on your computer after you link it to a cell phone number.
Your Spruce account is tied to your cell phone number. Please set up your Spruce accunt using your own name and phone number. If you have members on your plan who will not be managing their own Spruce account (young children or members who are unable to navigate the technology for any reason) then ask Dr. Rob to start a separate conversation thread for each of those members. That will help us keep conversations organized.
Connect with Still Point DPC on Spruce by using the link below or by scanning the QR code with your phone’s camera. After you have downloaded Spruce, send a quick hello to Dr. Rob through Spruce to let him know that you are connected!
Forms and Documents available for download
Medical Records Release Forms – Pre-filled records release PDF and blank records release PDF forms. Additional information about these forms is listed below.
Medicare Private Contract – This Medicare Private Contract PDF form should be signed annually by all clinic members who have any form of Medicare coverage. If you start receiving Medicare benefits in the future, then please submit a signed copy of this form once you receive your Medicare card.
Information Release to Family (optional) – This information release PDF form lists who we are allowed to talk to about your confidential medical information. Parents should sign for any members under age 18. Please note that HIPAA privacy laws allow discussion of your medical information with immediate members of your family when it is relevant to your health care, as long as you do not object (for more information on this rule, click here). This means that the main value of this form is to provide the clinic with a simple way to identify who is a member of your family (such as an adult son or daughter, especially when their last name may be different) when they request your medical information.
Additional information about medical records release forms:
To request a transfer of medical records, you will need to fill out a release of information authorization form. Use the pre-filled records release PDF file to request records from another clinic to be sent to Still Point DPC. This PDF file has Still Point DPC listed as the records recipient and requests all records from the delivering clinic from the most recent 2 years they have on file. If you need to request records to be sent from Still Point DPC to someone else, or if you need to request something other than the last 2 years of records from another clinic, you can download a blank version of the records release PDF here. You will need to:
- fill in your name and date of birth at the top of the form
- fill in the name and contact information for the clinic or doctor holding your records
- sign the second-to-last line at the bottom
Please return a signed copy to us for each clinic or facility holding your records in any of the following ways:
- send as an attached file through Spruce (preferred method)
- mail to Still Point Direct Primary Care, 1067 FM 306 Ste 106, New Braunfels TX 78130
- fax to 830-410-4752
- send by e-mail to DrRob@stillpointdpc.com
As a reminder, email is not a secure way to send sensitive information.
* You can ask the facility where your records are located to fax them to us at (830)-410-4752, although they usually still require some kind of signed consent to transfer records. Typically, we’ll just want your last 2 years of treatment records available.
Nutritional supplements
Dr. Rob has set up discounts for his patients’ supplement purchases through Fullscript. If you are interested in purchasing high-quality supplements from trusted suppliers using this discount, consider browsing the Fullscript catalog. In some cases, prices may be better through a large retailer like Amazon or Walmart, so be sure to compare before you make a purchase. If you have any questions about particular supplements, please bring it up during an appointment with Dr. Rob. Also, be sure to inform Dr. Rob about any supplements that you are taking (so these can be placed on your list of medications), as these can often interact with prescribed medications!
